How often will a nurse check on me?

Visit frequency depends on your condition and stability. Most chronic care clients receive weekly RN visits, with daily caregiver monitoring in between. Acute changes can trigger an immediate nurse visit.

Can you help me avoid the ER?

Yes. By catching warning signs early — weight gain in heart failure, dropping oxygen in COPD, rising blood sugar in diabetes — our RNs help you address problems before they require an emergency visit. We coordinate directly with your physician.

Do you coordinate with my specialists?
